Peripheral Nerve Inflammation and Central Pain:
Reflex Sympathetic Dystrophy
Symptomy
- Deep, grinding bone pain
- Crushing quality
- Does not correspond to any recognised area supplied by a sensory nerve
- Can travel along a limb
- Shoulder, next in the fingers etc.
- Can suddenly go, suddenly return [47]
- More severe form
- Accompanied by a redness of the limb
- Dilation of the superficial veins
- Increase in sweating in the limb
- Use of the limb may be limited
- Often an exaggerated perception of skin stimuli
- Light breeze may be felt as painful - seems transient :47::
- Painful tics
- Muscle fasciculations may also occur [47]
- Common in those with a chronic Chlamydia pneumoniae infection
- Many people develop it during bacterial killing
- Severity seems to depend on bacterial load and placement
- Usually mild
- Often does not last long
- Limb can remain red or mottled for some months
- Complete resolution is the rule [47]
- Severe thalamic pain of central origin can also occur in MS
- Usually long-lasting
- As described above
- Related to a bacteria-killing agent
- When metronidazole, for instance, is added to protein-synthesis inhibitors [47]
Etiologie
- Asi něco jako Sudeckův syndrom, jen ne v kloubu a po zjevném úrazu, ale jen tak
- Vznik/zhoršení po ATB terapii, následné vymizení
- Typical of damage to a large peripheral nerve
- Caused by injuries - often quite minor - to a nerve
- May occur after a virus infection [47]
- Chronic Chlamydia pneumoniae infection
- By acute inflammation associated with the destruction of a local bacterial collection (?) [47]
- Mechanism of the pain is not fully understood
- V.s. involve stimulation of the thalamus
- Pain is thus generated in the brain [47]
