Both tests evaluate the function of Mitochondrial Electron Transport Chain (ETC) complexes.

Completed Test Requisition Form signed by an authorized medical professional.
Completed forms may be faxed to 484-842-3400 or emailed to us at support@mitoswab.com
Once we receive the requisition form from your physician, we will ship the kit.
Return shipping is not included in the test kit pricing
We do not issue UPS labels for return shipping
ALL packaging and materials required are at the expense of the patient.
The sample MUST be received within 3 days and packed with a minimum of 10 lbs. of dry ice in a Styrofoam container

To keep the sample cool during transit (we cannot quote an exact amount of dry ice).

You will only receive swabs/vials, instruction sheet and a test requisition form.
You will need to arrange a Styrofoam box as well as dry ice.

Check with your local courier for the paperwork your country will need to release the shipment over to UPS/ FED EX.
If documentation is required at US Customs and Border Protection (USCBP), a commercial invoice has been attached to this email.
Kits are to be dispatched between Monday to Tuesday.
This is to ensure that we receive the sample in viable condition and a rejected sample will not be issued.
The sample should be packed with enough dry ice to keep the sample viable throughout transit.
