Mealworm (Tenebrio molitor) Anaphylaxis
A case report conducted in France
- Described a severe food anaphylaxis being induced by the mealworm (Tenebrio molitor) in a 31-year-old man
- Allergic to HDM but not to crustaceans, who consumed one cooked larva
- Prick-tests and a serum proteomic analysis
- Tenebrio molitor proteins to which he was sensitized
- Hexamerin,
- Tropomyosin,
- Alpha-amylase
- Previously identified as an allergen in mealworm with a structural homology with HDM
- Larval cuticle proteins A1A and A2B
- Both known mealworm allergens

Chemical hazards
- Heavy metals, mycotoxins
- Have to be quantified and evaluated (EFSA, 2015).

Direct sensitization
- Crucial aspect of insect allergies

Immunoglobulin E (IgE) cross-reactivity
- Between insects, crustaceous, and house dust mite (HDM) allergens
- Generally known as pan-allergens
- www.mdpi.com/2072-6643/15/14/3076
Tropomyosin and arginine kinase
- Have been found to be responsible for cross-reactivity between insects and other arthropods

IgE from canine sera sensitized to storage mites
- Bound to the protein in yellow mealworm
- IgE binding, however, does not mean that an allergic reaction will occur

Patients with shellfish allergies
- Allergic reactions after eating
- Cicadas
- Vegetable worms

Patients allergic to shrimp
- Double-blind placebo-controlled food challenge,
- 87% of patients allergic to shrimp and often also to dust mites
- Displayed allergy symptoms after eating yellow mealworm

Atopic dermatitis
- May affect 3% to 15% of dogs
- Dust mites are the allergens most commonly recognized by the circulating IgE of dogs with atopic dermatitis
- Use of insects as a protein source in dog food must be undertaken with caution.

Postharvest processing
- Thermal and high pressure processing
- Hydrolysis—of allergy-causing proteins
- Affect the allergenicity of feed
- May reduce their allergenicity and cross-reactivity

Microbiological risks
- Bacteria, fungi
- Have to be quantified and evaluated (EFSA, 2015).

Prevalence of allergic reactions to insects, and even death
- Considerably high
North-Eastern Thailand
- 2500 participants - 14.7% of them showed multiple symptoms of allergies after insect consumption

China
- Of all of the allergic causes for anaphylactic shock and fatalities in the collected Chinese literature, from 1980 to 2007
- 14% were attributed to locust and grasshopper ingestion

Toxicity studies
- Core areas of kinetics,
- Genotoxicity,
- Repeated dose toxicity testing (subchronic, chronic toxicity and carcinogenicity)
- Reproductive and developmental toxicity (EFSA, 2016).
- Consumption studies in the targeted species
