Mechanismus účinku
N-acetylcysteine (NAC) breaks disulphide bonds
- In Lp(a)
- Between homocysteine and plasma proteins
- V hlenu
- Ve stěně chlamydií aj. mikrobů
- Thiol-containing compound interfering with endogenous thiols
- Cysteine (Cys)
- Hcy
- By forming with them mixed disulphides
- Possibly more efficient renal clearance [7]
- Excess NAC v.s. displaces thiols from their plasma binding sites forming mixed disulphides.
- Urinary total Cys and Hcy excretion significantly increased at the end of the day of NAC infusion
- 10x for Cys
- 5x for Hcy [7]
- Reduced appreciably on the following day [7]
- Treatment with NAC and placebo was performed in a double blind study
- Treatment effects
- Plasma Lp(a) - no significant effect
- Plasma amino thiols - significantly reduced during treatment with NAC
- homocysteine - by 45%
- Cysteine - by 11%
- Cysteinyl glycine - by 24% [6]
- High dose of NAC was well tolerated [6]
